I checked out the details- there is nothing to send in- you simply fill out an online form with the following: bar code(s) of product(s)- seperate by commas if you have multiple items, date of purchase, price of item, place of purchase, and your shipping info. You can opt out of the magazine offer, and just get a "refund" for the value of $12 instead! In this case- you would have to print out the offer, and send it in with your reciepts for the refund.
